TRANSCRIPTS
Official
transcripts are normally required by an educational institution or an
employer. Two official copies are sent directly to the institution with
one student copy sent to the student at the same time as confirmation
of the request. The cost is $5.00 per institution. To make a request,
you must either come into the office and complete a form or make a written
request including the student's name and address, the name and address
of the institution, your Champlain I.D. number or your date of birth.
Requests will only be processed if they are accompanied by the $5.00
fee. We are unable to process requests by e-mail or fax.
STUDENT
COPIES OF TRANSCRIPTS
A
student's copy of a transcript for personal use may be obtained by coming
into the office or by mail. The cost is $3.00. the student's name, Champlain
I.D. number or date of birth is required. For confidentiality reasons,
if you request someone else to pick up a student's copy of your transcript,
we require that they bring a letter of authorization from the student.
Payment
may be made in cash or by personal check. Visa, Mastercard or Interac
is not accepted.
Every
effort is made to process request for official transcripts within three
days of receiving the request.
If the Official Transcript request is urgent, the transcript may be
faxed at an additional cost of $10.00 (local) or $15.00 (long distance).
Please be sure to check with your educational institution if a faxed
